Dabei wird versucht, auftretende Fehler als Lernchance zu nutzen, vorbeugende Maßnahmen zu ihrer Vermeidung zu setzen und damit zukünftig Sicherheitsrisiken zu minimieren. Um Fehlerpotentiale im Vorfeld zu erkennen, soll die Meldung von Beinahe-Fehlern ermöglicht und ein dafür erforderliches, vertrauensvolles Arbeitsklima geschaffen werden, das auf einer Unternehmenskultur und einer darin verankerten Fehlerkultur basiert, bei der nicht die Schuldfrage im Vordergrund steht, sondern die Faktoren und Begleitumstände im Arbeitsumfeld entlang einer ganzen Fehlerkette, die zur Fehlerentstehung geführt haben. Um das Personal zu freiwilligen Meldungen von Beinahe-Fehlern zu motivieren, müssen das Vertrauen, eine positive und anerkennende Kommunikation und auf den Betriebsablauf angepasste Meldeverfahren und -systeme entwickelt werden, die leicht anwendbar, überall verfügbar sind und die eine von der Führungslinie separierte, vertrauliche Behandlung mit einem konstruktiven Feedback ermöglichen. In contrast to aviation, railway companies quite recently began to deal with proactive error culture and just culture. Occurring errors shall be seen as a chance to learn from, to set preventive measures and thereby to minimize future safety risks. For previously recognized potential risks, it is necessary to enable the notification of near misses and therefore to create a good working atmosphere that is based on a positive corporate culture with an integrated, appropriate error culture. Based on such a culture, errors are not analysed to assign blame but to search for the specific factors and attendant circumstances that have enabled a whole chain of small errors that have led to the main mistake. To motivate the personnel to write voluntary reports of near misses or slips, the company concerned has to improve confidence, develop respectful communication as well as report processes and report systems that are established in accordance with the operating procedures.
