(Daniel) 2017 Reliabilität ; Ultraschall ; Feasibility-Studie Bachelorarbeit, Fachhochschule St. Pölten, Studiengang Physiotherapie, 2017 Einleitung: Um sinnvoll Forschung zu betreiben braucht es Messinstrumente, die valide und reliabel sind. In der Physiotherapie werden Werte bestimmt und gemessen um Evaluierungsparameter vor und nach der Therapie zu erhalten. Dazu werden Skalen und Messgeräte benutzt, die auf ihre Validität und Reliabilität getestet worden sind. Der Echtzeitultraschall ist derzeit ein kommendes Messinstrument und wird von immer mehr PhysiotherapeutInnen bei der Untersuchung und Evaluierung verwendet. Zuvor muss für eine Verlässlichkeit der Werte, die Reliabilität des Geräts getestet werden. Der Musculus vastus medialis wird aufgrund der Beteiligung bei Pathologien wie einer Patellaluxation oder dem Patellofemoralen Schmerzsyndrom für die Studie herangezogen. Methoden: Im Ablauf dieser Studie wird die Intertest-Reliabilität getestet. Dazu messen fünf TeilnehmerInnen je einmal mit dem Echtzeitultraschallgerät den Durchmesser des Musculus vastus medialis an der gleichen Stelle. Die fünf Ergebnisse werden darauf miteinander verglichen. Bei der Testung der Intratest-Reliabilität misst eine Person fünf Mal den Durchmesser des Musculus vastus medialis mit dem Echtzeitultraschallgerät. Die fünf Ergebnisse werden für die Intratest-Reliabilität verglichen. Die Auswertung erfolgt über das Programm SPSS Statistics Version 24. Ergebnisse: Die statistische Untersuchung ergab keinen signifikanten Unterschied. Die deskriptive Auswertung der Daten kann das Ziel weder verifizieren noch falsifizieren. Ein Konfidenzintervall von 95% schloss bei der Intertest-und Intratest-Reliabilität das höchste und niedrigste Ergebnis aus. Die Spannweite der Ergebnisse betrug bei der IntertestReliabilität 1,00cm und bei der Intratest-Reliabilität 0,16cm. Schlussfolgerung: Der Ablauf dieser Machbarkeitsstudie ist sehr gut durchführbar. Der Musculus vastus medialis ist aufgrund seiner Exponiertheit an der unteren Extremität optimal für die Messungen geeignet. Ein Schallkopf mit einer Breite von cirka 6cm ist zu empfehlen, da sonst nicht der ganze Musculus vastus medialis am Bildschirm abgebildet werden kann und in Folge die Genauigkeit der Studie gesenkt wird. Weiterführende Studien sollten mit zwei zu vergleichenden Gruppen durchgeführt werden um das statistische Auswertungsverfahren des Intraclass Correlation Coefficient anwenden zu können. Die Reliabilität des Echtzeitultraschalls muss mit zukünftigen Studien noch aussagekräftig belegt werden. Introduction: To conduct research in a practical way, the research needs measuring instruments that are valid and reliable. In physiotherapy the results have to be set and measured in order to gain evaluation parameter before and after the therapy. Scales and measuring instruments are used for testing the results for validity and reliability. Real-time ultrasound is currently the measuring instrument of the future and it is used by more and more physiotherapists during examinations and evaluations. Beforehand the reliability of the values therefore also the reliability of the measuring instrument has to be tested. Because of the muscle taking part in various pathologies like a patellaluxation or a patellafemoral pain syndrome it is consulted for this study. Methods: During the study the interrater-reliability is checked. For that each of the five participants measures the diameter of the musculus vastus medialis with the real-time ulturasound instrument. The five results are then compared. During the testing of the intrarater-reliability one person measures five times the diameter of the musculus vastus medialis with the real-time ulturasound instrument. The five results are compared for the intrarater-reliability. The analysis takes place via the SPSS Statistics Program version 24. Results: The statistical survey had no significant difference. The descriptive analysis of the data is not able to neither verify nor falsify the aim. A 95% confidence interval excludes the highest and lowest result during the intrarater and interrater reliability. The result-wingspan is 1,00cm during the interrater reliability and 0,16cm during the intrarater reliability. Conclusion: The process of the feasibility study is practicable. The musculus vastus medialis is perfectly suitable for measuring because of its exposure on the lower extremity. The transducer with the width of approximately 6cm is to be recommended otherwise only parts of the musculus vastus medialis would be seen on the screen and therefore the accuracy of the study would sink. Secondary studies should be carried out with two different groups in order to use the statistical analysis called Intraclass Correlation Coefficient. The reliability of the real-time ultrasound needs to be verified with future studies.
