o:2758 Einfluss von Kniebeugen auf instabilen Geräten auf das Aktivierungsmuster der Vastii bei ProbandInnen mit patellofemoralem Schmerzsyndrom de Bachelorarbeit, Fachhochschule St. Pölten, Studiengang Physiotherapie, 2015 Einleitung: Der M. vastus medialis (VM) gilt als wichtiger Stabilisator des Patellofemoralgelenks. Bei PatientInnen mit Patellofemoralem Schmerzsyndrom wird häufig angenommen, dass er schwächer und später aktiviert wird als der M. vastus lateralis (VL). In dieser Studie wird das Aktivierungsmuster der Mm. vastii bei ProbandInnen mit Patellofemoralem Schmerzsyndrom während beidbeiniger Kniebeugen auf verschiedenen Untergründen untersucht. Methodik: In dieser Studie wurden fünf ProbandInnen mit Patellofemoralem Schmerzsyndrom rekrutiert (4 weibliche, 1 männlicher, Mittelwerte ± SD: Alter, 22 ± 2,4 Jahre; Größe, 171 ± 5,9 cm; Gewicht, 65 ± 11 kg; BMI, 22,1 ± 2,6). Mittels kinesioloischer Elektromyogra-phie (EMG) wurde die Muskelaktivität des VM und des VL während beidbeinigen Kniebeugen auf stabilem Boden, instabilem Kippbrett und instabilem Ballkissen am stärker betroffenen Bein gemessen. Es wurden Daten bezüglich der Höhe der Muskelaktivität und bezüglich der Anschaltzeit erfasst. Zusätzlich wurden vor und nach den EMG Messungen Werte be-züglich der Posturalen Kontrolle erhoben. Als statistische Analyseverfahren wurden die ANOVA mit Messwiederholung, der t-test für abhängige Stichproben sowie der Friedmann-Test verwendet. Ergebnisse: Es gab keine signifikanten Unterschiede zwischen den verschiedenen Untergründen betreffend die Werte der mittleren Muskelaktivität, des Verhältnisses VM/VL und der Differenz der Anschaltzeit VL-VM. Eine Tendenz zur Signifikanz konnte während der exzentrischen Phase für das Verhältnis VM/VL zwischen stabilem Boden und Kippbrett erzielt wer-den, die auf ein besseres Verhältnis auf stabilem Boden hindeutet. Eine weitere Tendenz wurde in der konzentrischen Phase bezüglich der mittleren Muskelaktivität des VM festgestellt. Diese wird zwischen Kippbrett und Ballkissen vermutet und spricht für eine höhere Aktivität auf dem Kippbrett. Die Differenz der Anschaltzeit VL-VM zeigte am Kippbrett die früheste Aktivierung des VM, jedoch nicht signifikant. Außerdem scheint der VM auf keinem der Untergründe verspätet aktiviert zu werden. Schlussfolgerung: Die Messungen haben zu keinen signifikanten Ergebnissen geführt. Das beste Verhältnis von VM/VL konnte auf stabilem Boden erzielt werden, sodass dieser even-tuell in der Therapie von Patellofemoralem Schmerzsyndrom den instabilen Untergründen vorgezogen werden sollte. Für eine frühere Aktivierung des VM könnte ein Kippbrett als Untergrund dienen, jedoch sprechen die Ergebnisse nicht dafür, dass der VM bei Patellofemoralem Schmerzsyndrom verspätet aktiviert wird. Influence of Squats on unstable devices on the activation pattern of vastii in patients with patellofemoral pain syndrome Introduction: The M. vastus medialis (VM) is an important stabilizer of the patellofemoral joint. It is often assumed that in patients with patellofemoral pain syndrome this muscle has a reduced and delayed activity in contrast to the M. vastus lateralis (VL). In this study the acti-vation pattern of the Mm. vastii in subjects with patellofemoral pain syndrome during two-legged squats on different surfaces is analyzed. Methods: Five subjects with patellofemoral pain syndrome were recruited for this study (4 female, 1 male, mean ± SD: age, 22 ± 2.4 years; height, 171 ± 5.9 cm; weight, 65 ± 11 kg; BMI, 22.1 ± 2.6). Using kinesiologic Electromyography (EMG) the activation of VM and VL muscle during two-legged squats on stable ground, wobble board and rubber air disc on the affected leg was measured. Data was collected regarding the amount of muscle activity and the onset time. In addition, before and after the EMG measurements values regarding the Postural control were collected. Repeated measures ANOVA, paired t-test and Friedman test were used as statistical analysis methods. Results: No significant results concerning the mean muscle activity, the ratio VM/VL and the difference of onset time VL-VM were found. A tendency to significance was reached between stable ground and wobble board during the eccentric phase for the ratio VM/VL. This would mean the most simultaneous ratio VM/VL can be reached on stable ground. Another tendency was observed in the concentric phase regarding the mean muscle activity of the VM, which is assumed between wobble board and rubber air disc and tends to higher activity on the wobble board. The difference between the onset times showed the earliest activation on wobble board, but not significantly. In this study the electromyographic onset of the VM doesn’t appear delayed on any surface. Conclusion: No significant results were found. The best ratio of VM/VL was reached on stable ground. This indicates that it could be preferred in the treatment of patellofemoral pain syndrome. For an earlier activation of the VM a wobble board could be used as surface, but the results do not indicate that the VM is activated delayed in patients with patellofemoral pain syndrome.
