Schwierigkeiten und Verbesserungsmöglichkeiten im physiotherapeutischen Erstgespräch bei Jugendlichen im NEET-Status mit Migrationshintergrund

Einleitung: Bei Personen mit Migrationshintergrund stellt die Kommunikation im Erstgespräch zwischen PatientIn und medizinischem Fachpersonal häufig eine Herausforderung
dar. Diese Arbeit beschäftigte sich im Besonderen mit Schwierigkeiten und Verbesserungsmöglichkeiten im Erstgespräch bei der Zielgruppe Jugendliche im „not in employment, education or training“ (NEET)-Status mit Migrationshintergrund.
Methodik: In dieser Studie wurden qualitative leitfadengestützte Fokusgruppeninterviews
durchgeführt. Eine Gruppe umfasste vier ExpertInnen und die andere vier Jugendliche. Es
gab einen Messzeitpunkt, in welchem zuerst eine Befragung mit den Jugendlichen stattfand. Diese behandelte die Thematiken Schwierigkeiten und Verbesserungsmöglichkeiten
im Erstgespräch bei Jugendlichen im NEET-Status mit Migrationshintergrund. Nach dieser
gab es bei den Jugendlichen, anders als bei den ExpertInnen, noch einen zweiten Teil der
Befragung. In diesem wurden medizinische Begriffe für die App mobile ICanFunction
(mICF) in Jugend- beziehungsweise (bzw.) Laiensprache übersetzt. Anschließend wurden
die Jugendlichen verabschiedet und nach einer Pause fand die identische Befragung über
Schwierigkeiten und Möglichkeiten zur Verbesserung mit den ExpertInnen statt. Die Datenauswertung erfolgte mittels strukturierender Inhaltsanalyse nach Mayring.
Ergebnisse: Die Ergebnisse der jungen Generation waren mit den in der Literatur beschriebenen Hauptthemen für Schwierigkeiten in einem Erstgespräch beinahe ident. Die
Hauptproblematik stellt das Nicht-Verständnis aufgrund mangelnder Deutschkenntnisse
oder zu hohen Einsatzes an Fachbegriffen dar. Die ExpertInnen sehen das zu hohe
Sprachniveau und die Anzahl der Fragen in einem Gespräch als kritische Punkte. Ein
bewährtes Hilfsmittel bei Verständigungsschwierigkeiten in einem Erstgespräch ist für alle
TeilnehmerInnen die nonverbale Kommunikation. Zukünftig würden sich die Jugendlichen
bei Sprachbarrieren als Hilfsmittel eine geeignete App wünschen, wobei die ExpertInnen
momentan von technischer Hilfe Abstand halten. Bei der Übersetzung der Begriffe für die
App konnten viele Begriffe für die Weiterentwicklung dieser gewonnen werden.
Schlussfolgerung: Einige auftretende Schwierigkeiten und Verbesserungsmöglichkeiten
konnten aufgezeigt werden, jedoch besteht bislang keine optimale Lösung zur Erleichterung des Erstgespräches, weshalb weitere Studien über die Effektivität der aufgeführten
Möglichkeiten durchgeführt werden sollten.
Introduction: In people with migration background, the communication in the initial consultation between patients and the medical staff commonly poses a challenge. This study
focuses in particular on the difficulties, as well as the options for optimisation of the initial
consultation with adolescents of “not in employment, education or training” (NEET)-status
and with migration background.
Methods: In this study, qualitative guideline-based focus group interviews were carried
out. One group was made up of four experts, while the other group was made up of four
adolescents. There was a measurement time point, at which an initial interview with the
adolescents took place. The subjects difficulties and options of optimisation of the initial
consultation with adolescents of NEET-status and with migration background were covered. Following that, the adolescents had, unlike the experts, a second part of the interview
in which specific medical terms for the app mobile ICanFunction (mICF) were translated
into youth or layman language. Then the adolescents were dismissed, and after a break,
the same interview about the difficulties and options of optimisation was conducted with
the experts. The analysis of the data was carried out through structured content analysis
as described by Mayring.
Results: The results of the young generation were almost identical to the difficulties in an
initial consultation described in the literature. The main problem was found to be the lack
of understanding due to insufficient knowledge of the German language or the frequent
use of technical terms. The experts see the language level that is too high, as well as the
amount of questions asked during a consultation as critical points. A proven strategy
against communication difficulties in an initial consultation is the non-verbal communication between all conversation partners. For the future, adolescents would like to have a
suitable app that provides support with language barriers, even though experts currently
keep away from technical support. During the translation of the terms for the app, some
terms could be used for the further development of the app.
Conclusion: Several occurring difficulties and optimisation options could be demonstrated, however, there is no optimal solution for the simplification of an initial consultation
yet, which is why further studies of the effectiveness of the stated options should be carried out.
